We collect the details you volunteer — name, phone, email, state, and what you would like help with — when you request an appointment or finish a screening and ask to be contacted. Screening answers you do not submit stay in your own browser.

How we use it

• To contact you about the appointment you requested
• To verify insurance benefits before your visit
• To meet legal, licensure, and record-keeping obligations

What we do not do

We do not sell your information, and we do not share it with advertisers.
